Network Install - Sample Scope of Work

Since the overriding principle at PCSI is providing the customer with easy to understand information we have several systems in place to provide you with this information. One example of this information is our Scope of Work document. Instead of providing you some vague quote that leaves plenty of room to "hide" hidden costs, PCSI gives you a play by play scope of work document detailing what will be done for the investment you are making. Every job we do for you will be prefaced with a Scope of Work that will be reviewed with your sales representative and/or assigned engineer before we do the job. We make sure you are comfortable with all aspects of the project, big or small. Included below is a sample scope of work.

Overview:

 

Current customer concerns are:

  • Ensuring secure connections between offices.
  • Coordinating with the Medical Software Vendor for software install.
  • Ensuring all locations have access to new medical software..

Scope of Work Requested:

PCSI Customer has requested Princeton Computer Support, Inc install and configure a new Wide Area Network (WAN). The WAN will consist of 3 offices. The three locations consist of Location 1, Location 2 and Location 3. The central office is located in Location 1. The Location 1 office consists of 3 floors. The first and third floors are comprised of office space while the second floor is a surgical center. The server will reside at this location. The server will be a tower style IBM X-Series 240. This server will be placed on the floor or on a table of the customer's choosing. There will be 25 workstations installed and configured in Location 1. These workstations will have Windows 2000 Professional and be networked via 10/100 Ethernet. Location 3 will consist of 4 Workstations with Windows 2000 networked via 10/100 Ethernet. Location 2 will consist of 3 Workstations with Windows 2000 networked via 10/100 Ethernet. Each location will be wired for CAT 5 Ethernet and configured with a 10/100 3COM Hub. All workstations will be configured to log in to the server located in Location 1. PCSI will configure the Network to be a Windows TCP/IP based network addressed in the 192.168.111.0/24 (Location 1), 192.168.112.0/24 (Location 2), and 192.168.113/24 (Location 3) networks. TCP/IP will be the only protocol installed.

PCSI will install and configure the Health-IS AdvantX medical support application on all computers accross the WAN. AdvantX security will be handled independantly from system security. Symantec Anti-Virus Corporate Edition will be installed and configured. To allow for updates of the Anti-Virus Definitions the network MUST have access to the internet. In the event that internet access is not available, the customer will be responsible for keeping the Anti-Virus definitions up to date. All users will be assigned network accounts and Email boxes. The server will be configured to automatically backup itself
up every night at 11:30pm. The customer should change the backup tapes daily. Microsoft Office XP will be installed on each workstation with the default installation options. PCS will configure and install the 3 Cisco Routers that will be used for the
WAN connections. In the event that the internet is used , PCSI will additionally configure the Watchguard SOHO Firewall to protect all three locations from internet threats.

No other software will be installed or configured.

Customer Requirement's:
All assumptions MUST be correct to validate this scope of work. If anything listed below is inaccurate contact Princeton Computer Support immediately. Any inaccuracies will result in increased labor.

 

  • The Server and workstations MUST be placed within 6 Foot of a 110volt grounded outlet.
  • The Server and workstations MUST be placed within 10 Feet of a HUB or CAT5 Network drop location.
  • The server MUST be placed in a secure, well ventilated area.
  • Internet access is REQUIRED for automatic Anti-Virus Updates
  • Labor will be billed per man-hour plus travel.
  • Customer MUST be properly licensed for all software that will be installed.
  • The Customer is responsible for coordinating with the local loop provider and/or the ISP to ensure that T1 connectivity is available between the offices BEFORE the install date.
  • Full time 24/7 network internet access is required to send and receive internet email. If the internet is not available, the customer will only be able to E-Mail within the WAN (Between offices).
  • The Hubs, Routers and Firewalls will need to be installed on a shelf or mounted to the wall.
  • These devices must be placed within 4 feet of a grounded 110Volt outlet.
  • There is no rack system or hardware quoted. All network electronics MUST be mounted on shelves or on the wall. Racking systems are available for an additional cost. Contact your sales rep if you are interested in this option.

Customer Instructions:

Please carefully review this document. The dispatched technician will ONLY be authorized to perform work that is specifically listed in the above scope of work. Any additional work will need to be scheduled as a future service call by executing a Change Order. The customer is responsible for having access to all software currently installed on their PC's. Failure to have CD's for Floppies can significantly increase the time spent onsite and could make completing the service call impossible. The customer should have access to all required hardware prior to the install date. Any revisions to this Scope of Work must be recorded by execution of a Change Order.

- Business Hours are Monday-Friday 8:00am to 6:00pm (excluding holidays).

 

To accept this scope of work, please sign & date below where indicated. Fax the Scope of work document to (609) 921-7691. To ensure quick response time, after faxing please call (609) 921-8889x107 and speak to the PCSI dispatcher. Reference the above scope of work number and ask
to schedule a service call. At the completion of this scope of work you will be asked to sign as to its completion. No additional service call can begin until this scope of work is complete. Please keep original at your location. The onsite technician will pick it up. At the completion of the scope of work, a copy will be attached to your invoice.

 

By signing below you are agreeing to the terms and conditions listed in the scope of work above. No changes or additions to the scope of work will be accepted after the date that appears below. Additional work MUST be scheduled as a separate service call by executing a Change Order.
